· Salary of £22,276.80 per annum - 34 hours per week

Contract:
The Service is aimed to respond to people aged 16 plus that present with complex care and support needs. It involves delivery of Intensive Complex Care and Support to individuals in their own homes or accommodation sourced by Humbercare, many stepping down from long term hospital or residential placements, those leaving the care system or secure or prison stays; working to develop people’s capacity to live independently.
Key Responsibilities:
· To carryout needs and risk assessments, conduct support and care plan reviews and provide care and support to all customers accepted onto the Self Enablement Service.
· To engage with customers on an agreed basis to support individuals to achieve and maintain independent living.
· To provide information, advice and advocacy on welfare rights, housing options, tenancy rights and responsibilities, with a focus on helping customers develop skills and knowledge in these areas.
· To provide information and assistance to customers to enable them to make choices about the development of their social network and participation in their local community, including employment, education, and training opportunities.
· To work with customers to identify support needs which can be met by other agencies, to make referrals to specialist agencies, and develop appropriate casework links with such agencies in relation to customers' support plans.
· To provide care and support in accordance with current best practice, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Registered Manager.
· To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers. Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives.
