This personalised housing related support is available for people with learning disabilities and/or autism in Hull and North East Lincolnshire, helping people develop independent living skills and achieve access to community resources.

Who is this service for?
Commissioned by Hull City Council in Hull and the North East Lincolnshire Clinical Commissioning Group in Grimsby these support services are dedicated to responding to individuals with learning disability and/or autism who require assistance in developing skills, resilience, and pathways towards greater independence.
These low-level, floating support services are designed to enable people to develop sufficient independent living skills to sustain a tenancy, move towards greater independence or remain living independently in the community without the need to access long term residential care. Our support services include assistance with cooking, budgeting, life skills, establishing good links within the community, and helping individuals access work and training. We provide personalised, wraparound support that builds on strengths and assets to achieve the outcomes that matter to our service users.
Our approaches to support are based on Making Every Adult Matter (MEAM), Psychologically Informed Environments (PIE), and Trauma Informed Care (TIC). We also work with a number of key partners within the locality to provide critical and complementary services for our service users to ensure positive outcomes are achieved.
Humbercare is a registered personal care provider under the Care Quality Commission. Along with the housing-related support package, the Learning Disability HRS service in Grimsby also offers a care service to service users in cases of an assessed need.
